These Terms of Use (“Terms”) govern your use of drbradleymartin.com (the “Site”) and any related

content, scheduling, communications, or services provided by Dr. Bradley G. Martin (“we,” “us,” “our”).

By using the Site, you agree to these Terms.

1) Not Emergency Services

Do not use this Site for emergencies. If you have an emergency, call 911 (U.S.) or your local

emergency number.

2) Educational Services; No Medical Treatment

Unless explicitly stated in a separate written agreement and permitted by law, services and

consultations offered through this Site are education-based and are not a substitute for medical

diagnosis or treatment. The Site content is for general informational and educational purposes only.

3) No Provider–Patient Relationship Through the Site Alone

Using this Site or sending a general inquiry does not create a provider–patient relationship. Any formal

relationship (if offered) is established only after intake, informed consent, and confirmation of services

through appropriate channels.

4) Telehealth and Jurisdiction Limits

If telehealth or remote services are offered, availability depends on your location and applicable laws,

including licensure and scope-of-practice requirements. We reserve the right to decline or discontinue

services if we cannot legally provide them in your jurisdiction.

5) Scheduling, Fees, Payments, Cancellations, Late/No-Show

If you schedule or purchase services:

• Fees are disclosed at the time of booking or purchase.

• Payment may be collected in advance and processed through third-party processors.

Cancellation Policy (24 hours):

Cancellations must be made at least 24 hours before the scheduled appointment time. Late

cancellations forfeit the right to a refund, but may be eligible for rescheduling at our discretion.

Late Policy:

Arrivals more than 10 minutes after the appointment start time are considered late and will be required

to reschedule.

No-Show Policy:

No-shows are charged the full fee for the visit.

We may update offerings, availability, and pricing at any time.

6) Supplements, Herbs, and Products

If you choose to use supplements or herbal products:

• You are responsible for disclosing relevant health information, conditions, and medications.

• Supplements/herbs may carry risks and potential interactions.

• Third-party pharmacies/dispensaries have their own policies (shipping, returns, etc.).

7) User Conduct

You agree not to:

• Use the Site for unlawful, abusive, or harmful purposes

• Attempt unauthorized access to systems or accounts

• Interfere with Site operation (malware, scraping, automated attacks)

• Post or transmit content that infringes rights or violates laws

To the fullest extent permitted by law, we are not liable for indirect, incidental, special, consequential, or

punitive damages arising from or related to your use of the Site or services. Our total liability for any

claim related to the Site will not exceed the amount paid to us in the prior 3 months, or $100 if you paid

nothing.

You agree to indemnify and hold us harmless from claims, liabilities, damages, costs, and expenses

(including attorneys’ fees) arising from your use of the Site, your violation of these Terms, or

infringement of rights.

13) Privacy

Your use of the Site is also governed by our Privacy Policy.

15) Governing Law and Venue

These Terms are governed by the laws of the State of Illinois, without regard to conflict-of-law
